Have you tried Microsoft Gallery online? They have various templates available.

Microsoft Templates have just about everything from flyers, memos, newsletters, accpunting, etc. Just go to microsoft.com and click templates

Hi,
www.ezcontactform.com will allow you to create the form and automatically have it sent out by the user. It will also download the completed forms forms into an excel file. Pretty cool. |
