Please help!

I’m making a very simple form ex. which of the following would you like grapes
apples
oranges each has a checkbox after it
I would like to send it so the receiver only has to “x” the box and send back
I can sent it to myself but could not “X” the box or return it

I am using word 16 and I have adobe pro dc (do I need it for this project?) If I could not use adobe that would be ok by me
