Q: Are amounts paid for wellness programs and on-site medical clinics reportable in Box 12 of employees’ W-2s? Also, can you explain when dental and vision plan costs are reportable on Form W-2? What qualifies these benefits as “separate” plans?

A: If employers don’t charge COBRA beneficiaries a premium to use wellness programs and on-site medical clinics, W-2 reporting isn’t required.

Dental and vision benefits that aren’t offered under the major medical policy are offered under a separate plan; those benefits also aren’t reportable on employees’ W-2s.
